HOW TO APPLY FOR YOUR MEDICAL MARIJUANA PATIENT LICENSE IN OKLAHOMA

Woohoo! State Question 788 passed and now you're ready to start your journey to the height of your personal wellbeing, right? Well, before you go skipping off to your nearest dispensary, you'll want to know more about how to apply for your license, the rules and regulations surrounding State Question 788 and the Emergency Provisions for implementation dubbed Chapter 661 that the Oklahoma Department of Health has passed. We're here to help you learn more about patient licensing, what medical marijuana is, and how you can use marijuana with your doctors recommendation to supplement your other treatments.

We've made the guide for applying for your medical marijuana patient license in Oklahoma easy. Just follow these steps below and you'll have your license in no time! The State of Oklahoma asks for a few different pieces of information before you can submit your application. While getting together the information required can be a project, the information that the Oklahoma Department of Health requests is entirely fair and will help create a system that is not abused for Medical Marijuana patients in Oklahoma. The good news is that your license will last for 2 years. The bad news is that you'll (unless changed) have to apply in the same way you did the first time.


STEP 1: EVALUATE YOUR SYMPTOMS & VISIT YOUR PHYSICIAN

The first step in assessing your eligibility for becoming a Medical Marijuana Patient is to visit your healthcare provider and discuss your symptoms. Without a license that your physician recommends, you won't be eligibile to purchase Medical Marijuana in the state of Oklahoma. If ineligible, CBD is still a fantastic option.



STEP 2: ACQUIRE THE OKLAHOMA HEALTH DEPARTMENT ISSUED FORM

Contrary to popular belief, a simple letter from your doctor stating that you qualify to become a Medical Marijuana patient is not all you need to begin purchasing marijuana in Oklahoma. You'll be applying with the Oklahoma Health Department for your Medical Marijuana patient license and providing them with the information they need to make sure you're a qualified patient.

STEP 3: GET THE APPROPRIATE INFORMATION TOGETHER (APPLICANTS 18+)

You're almost to the peak of the mountain! After this, it's all downhill. Our list is taken directly from the final draft of the Emergency Provision outlined in Chapter 661. The requirements start on SUBCHAPTER 2. MEDICAL MARIJUANA LICENSES under 310:681-2-1. If that's too much, just go to page 10. We're only listing the non-basics below and you'll be able to see the full list on the form or in Chapter 661.


  1. Affidavit of lawful presence form

  2. (ONE) Document establishing you are an Oklahoma Resident, including: (A) Unexpired Oklahoma issued driver's license (B) Oklahoma voter ID card (C) Utility bill for the calendar month preceding the date of application, excluding cellular and internet bills (D) Residential property deed (E) Current rental agreement of residential property in Oklahoma

  3. (ONE) Document establishing proof of identity, including: (A) Front and back of an OKlahoma Driver's License (B) Front and back of an Oklahoma ID Card (C) US Passport or other photo ID issued by the Federal Government (D)Certified copy of your birth certificate (E) Tribal ID card that has been approved by the OKDPS

  4. A digital photograph that is: (A) Clear, color photograph of the head and top of shoulders (B) .jpg, .png, or .gif format no larger than 3 MB in size (C) Be in either a scanned photo format of a 300 pixel resolution per inch from a 2x2 inch image with dimensions in a square aspect ratio OR a captured image of 600x600 pixels and a maximum acceptable pixel dimension of 1200x1200. (D) Taken within the last 6 months (E) Taken in front of a plain white or off-white background (F) Be taken in full-face view directly facing the camera at eye level with nothing obscuring the face. If you do wear these for religious purposes, submit a signed statement verifying it is such. If you do wear one for medical purposes, submit a signed doctor's note. (G) Take the photo with a neutral facial expression or natural smile with the mouth closed and both eyes wide open (H) No digital enhancements (I) Sufficiently resemble proof of identity and residence

  5. Certification and recommendation from an Oklahoma Board Certified Physician dated within 30 days

  6. Your physician's name & license number including an ID of the physician's license type and area of board certification

  7. Office address on file with the physician's licensing board

  8. Patient/applicant's date of birth

  9. Physicians signed and dated attestation of (A) physician has established an ongoing relationship with you and knows your medical history (B) Physician has conducted an in-person physical exam within 30 days (C) Physician has discussed risks/benefits (D) Physician has determined presence of medical conditions calling for use of marijuana (E) Patient is recommended a medical license (F) The information provided by the physician is true and correct (G) State the method by which the physician verified the patient's identity (310:681-1-7) (H) Physician has met continuing medical education requirements as requried by their licensing board.

  10. Payment of the application fee, unless you are insured by Medicaid, Medicare, or SoonerCare (may change, check http://omma.ok.gov/ website)

  11. If you are insured by any of the aforementioned, you must provide the following: (A) copy of your insurance or other acceptable verification (B) They will verify you are actually on one of those, so make sure your insurance agent likes you (C) You'll pay a reduced application fee

  12. Remember: all application fees are nonrefundable

  13. Application must be submitted within 30 days of signature or it will be rejected

STEP 4: INFORMATION FOR APPLICANTS BELOW 18

If you're 18 or older, skip this step. If not, you'll have to do everything from step 3 plus step 4.


  1. You must have the recommendation of 2 physicians, both of whom must be either a pediatrician or pediatric subspecialist, dated within 30 days of eachother who do not practice together or who are not otherwise in a business relationship. Both of them have to recommend you for the exact same thing.
  2. Application must be completed listing the minor as the applicant and your parent/guardian has to supply the information from step 3 about themselves.
  3. Signature and date of each parent or legal guardian must be included on the application. If you can't have both parents, you have to have a notarized affidavit stating the reasons the parent/guardian cannot sign. As long as it's not refusal or disagreement, you'll be fine.
  4. Attestation by the parent/guardian that the information is correct
  5. You might not be able to smoke/vape marijuana, so keep that in mind.

STEP 5: SUBMIT YOUR APPLICATION
